1Clinical Functions-1. Early detection, screening and first level management.  Identify and provide first level management -12 essential package of services.  Communicable disease screening –TB, leprosy, Filariasis etc.  NCD Screening – Hypertension, Diabetes, Cancer Breast, Cervix and Oral 2. Undertake referral to enable continuum of care  High Risk cases, complications, medical emergencies  Guidance for Contraceptive methods/limiting methods.  Children 0-18 years –assessment for 4Ds  Cases with tertiary/secondary care under PMJAY

2Facilitate Teleconsultation  . Community level –for health promotion

3 Public Health Functions 1. Collection of population –based data and planning of services at HWC  Listing population/households, maintain database .  Supervise data collection by frontline workers, collation and analysis of population data  ‘Understand and act’ on demographic process like- Fertility, mortality, marriage, migration and social mobility 2. Disease Surveillance  Continuous vigilance towards agent, host and environmental factors affecting disease outbreak/epidemic.  Verify and inform authorities of response to disease outbreak.  Support the medical team or joint investigation teams for disease outbreak.
