As a Clinical Attaché, I sought to become acquainted with professionalism in the NHS, observe the work of a multidisciplinary team, and gain experience in NHS operations. My tasks included:

• Participating in daily ward rounds with consultants, including acute and post-take rounds.
• Observing post-operative orthopedic patients in the wards and providing ward cover.
• Liaising with on-call SHOs, clerking patients, and placing them on the theater list.
• Learning to order investigations, chase reports, make referrals, and fill in eDNs via software in the wards.
• Attending MDT meetings and clinical governance sessions.
• Actively contributing to clinical audits.
• Observing various hip and knee replacement surgeries.
• Undertaking clinical courses offered by the trust to enhance safety, minimize risk, and develop effective coping strategies for pressure and challenges.

Please note, as this was an observational post, I did not perform any exposure-prone procedures or have direct patient contact.

-I was working as a medical officer (SHO level).

- Conducting initial patient assessments, including history-taking and physical examinations.

- Assisting in surgical procedures, such as preparing the operating room and providing support to the senior doctors.

- Participating in ward rounds to monitor patients' progress and update treatment plans.

- Providing general patient care, including prescribing medication and recommending rehabilitation programs.

- Documenting patient records accurately and maintaining confidentiality.

- Collaborating with the multidisciplinary team, including physiotherapists, nurses, and other healthcare professionals.

- Responding to emergency situations and providing immediate medical care.

- Attending educational sessions and training programs to enhance medical knowledge and skills.

- Adhering to ethical and professional guidelines, prioritizing patient safety and well-being.

• Worked under Dr. Rifaz in the Emergency Department.
• Handled emergency clinical procedures and managed sick patients.
• Stabilized patients and provided wound closure.
• Managed minor trauma cases and interpreted X-rays and ECGs.
• Performed catheterization and inserted nasogastric (NG) tubes.
• Administered and managed emergency drug doses.
• Assisted in aligning hip and shoulder dislocations.
• Attended Multi-Disciplinary meetings and managed routine cases in the outpatient department.
• Taught and supervised clinical observers.

• During my internship, I had rotations in the following departments - General Medicine including 15 days of Psychiatry (2 months), Community Medicine (2 months at Yenepoya medical college mangalore) , General Surgery including 15 days of Anaesthesia (2 months), Obstetrics and Gynaecology including Family Welfare Planning (2 months), Paediatrics (1 month), Orthopaedics including Physical Medicine and Rehabilitation (1 month), Otorhinolaryngology (15 days), Ophthalmology (15days), Casualty (15 days) and Respiratory Medicine / Dermatology (15 days). Prior to the beginning of my rotations, I attended a pre-internship training program organised by the consultants and staffs of various departments. The orientation program included Role and Limitation of a Junior Doctor, Patient Safety, Basic Life Support Training, Importance of developing Interpersonal skills and maintaining a good doctor - patient relationship, Universal Safety Precautions and Post-Exposure Prophylaxis, Hospital Waste disposal etc. My duties and responsibilities were:
• Actively participating in ward rounds with consultants, registrars, post-graduate students, house officers and nurses.
• Adequate patient clerking.
• Detailed history taking and clinical examination of both out-patients and in-patients which included emergencies.
• Initial plan of management after preparing comprehensive differential diagnosis.
• Doing various procedures including Venepuncture for Blood sampling and Blood culture, Arterial Blood Gas sampling, Intravenous cannulation, Nasogastric tube insertion, Urethral Catheterisation of both males and females, Preparation and Administration of IV, IM, Subcutaneous and Intradermal injections including routine immunisations, Wound care and suturing, Intravenous infusion of fluids, blood and blood products.
• Perform and interpret ECG and peak flow.
• Documentation and record keeping like typing patient notes, prescriptions, referrals and discharge summaries. Teaching medical students regarding case presentations and care.
• Conducted multiple normal vaginal deliveries.
• Assisted various major and minor surgeries.
• Had on-calls, weekend shifts and night shifts.
• Airway management, oxygen delivery and nebulisation.
• During my training, I was able to observe, assist and perform (under supervision) procedures like Lumbar puncture, Spinal Anaesthesia, Central Venous catheterisation, Pleural Tap, Ascitic Tap, Endotracheal intubation, oxygen delivery and nebulisation.
