With a Major in Psychology and Minor in Biomedical Sciences from Xavier University, I have been able to thrive and excel in my postgraduate work. Through my career at Oak Street Health, I have been able to work along side primary care providers in the clinical setting, advancing my knowledge and learning what it takes to care for our community. As a Direct Support Professional, I am able to work hands on with those in need, turning my knowledge and experience into positive outcomes for those less fortunate than I. Because of these experiences and many others, I look forward to one day becoming a medical professional, working with others to improve the health and wellness of our society.
- Work directly with leadership including Medical Directors, Population Health Director, and Senior Clinical Informatics Specialists
- Audit clinical documentation for compliance and adherence to clinical guidelines surrounding diagnosis and amendable language for all of Indiana and Kentucky regions
- Provide support for providers and scribes during quarterly clinical documentations reviews
- Gather, track, and review clinical documentation trends utilizing Tableau, Excel, and other data systems
- Complete frequent education seminars in order to stay up to date with diagnosis criteria, proper disease workup and treatment, as well as correct coding and documentation practices
- Support primary care providers with clinical documentation
- Assist providers throughout the patient care journey — huddling each morning to plan for the day’s visits, joining them in the exam room to observe and document, and touching base after the visit to assist with next steps.
- Become experts in the value-based care model and the documentation and care of chronic conditions, including ICD-10 and CPT coding
- Receive extensive on-the-job training in clinical workflows, value-based medicine, preventative care for chronic conditions, accurate and specific documentation, population health data streams, and team based care
- Identified behaviors that necessitated intervention and obtained assistance from qualified personnel to complete intervention
- Supported patients needing medical care to relieve patient anxiety and promote positive outcomes
