By education, I have completed Bachelor of pharmacy + Doctor of pharmacy (post baccalaureate) from Poona College of Pharmacy, Bharati Vidyapeeth Deemed University, Pune, India. Clinical study have been done in antibiotics appropriate use in pediatric department for the period of 2 years and its has been published. Have experienced 2 times NABH pre entry level audit with auditors and currently going for nursing excellence in current organization. Clinical experience, actively participating in ward rounds with physicians, reviewing patients medication chart along with history taking and providing pharmaceutical care services such as, Medication therapy management, Patient discharged medication counseling, Avoid duplication of therapy, Pediatric dose calculation, dose adjustment based on liver and renal function, Detecting and reporting of ADRs to pharmacovigilance center, Drugs & Poisons Information to healthcare worker, Designed and implemented Medication reconciliation to assure safe patient treatment, Identify Drug-Drug interaction and Medication errors along with necessary corrective and preventive action (medical intervention), NABH standard and safe prescription audit. Antimicrobial stewardship program- Formulated and developed antibiotic policy by using antibiogram and standard guidelines, Newly designed and implemented Justification of high end antibiotics form, Involve in clean cases prophylaxis audit for appropriate antibiotic usage, comment on escalation and descalation of antibiotics. All finding discussed in Hospital infection control meeting every month. Certified with Master class in NABH implementation and having good knowledge and understanding of the NABH standards (5TH edition) by ACME Consulting. A very good learner and can adapt to given situations quickly. Also an effective communicator, hardworking, responsible, leadership quality and efficient in grasping new pharmaceutical concepts and utilizing them in an effective manner.
