Roles And Responsibilities

  • Prescription audit - To check prescription for completeness and appropriateness.
  • Communicate with concerned doctors for appropriateness of therapy.
  • Review patient medication history.
  • Identify, prevent, report and perform CAPA for medication errors.
  • Check for potential and significant drug-drug and food-drug interaction.
  • Pharmacovigilance - Monitor, assess, report and document any adverse drug reactions.
