Resident Medical Officer

  • A full time Medical officer role in a acute care hospital working across rotational shifts of 50 hours per week.
  • Managed a 20-bed Level 2 High Dependency Unit (HDU), including post-operative and tracheostomy patients, working closely with ICU consultants.
  • Functioned as the first responding medical officer during acute emergencies and resuscitation codes, initiating immediate assessment and management in high-pressure clinical situations.
  • Attended daily ward rounds and collaborated with senior clinicians in formulating and reviewing patient management plans.
  • Performed clinical assessment of newly admitted patients and formulated initial diagnostic and management plans.
  • Gained hands-on exposure to clinical and procedural skills, like airway intubation, resuscitation, and assisted ventilation.

Core responsibilities across rotations included initial patient assessment, daily ward rounds with case presentation, clinical documentations, basic procedural skills, teaching sessions, patient monitoring and escalation of care under consultant supervision within a multidisciplinary team.

1) Adult Medicine -

  • Comprehensive history-taking and examination, and formulation of differential diagnoses.
  • Interpretation of laboratory and imaging, evidence-based medicine.
  • Procedural skills like IV line insertion and sampling, ABG analysis, and paracentesis.
  • Development of care plans and coordination of interdepartmental referrals.

2) Adult Surgery -

  • Hands-on exposure to core surgical skills like suturing, wound dressings, and ulcer care.
  • Performed bedside minor debridement, fasciotomy, I & D, and suturing under supervision.
  • Assisted in major procedures like meshplasty, laparotomy, and thyroidectomy.
  • Performed skin-to-skin procedures, such as lipoma excision, I&D, and amputations, under supervision and guidance. Observed laparoscopic procedures.

3) Acute and Critical Care -

  • Responsibilities include triage, primary survey, resuscitation and vital monitoring.
  • Assisted with and observed rapid responses to Code Blue, Code STEMI, Code Stroke, and MET calls.
  • Performed minor skills like IV line, ABG, Bag mask ventilation, ECG and POP applications.
  • Observed and assisted consultants in central line insertion, arterial line insertion, HD catheter insertion, interosseous line insertion, and ICD tube insertion.

4) Obstetrics and Gynecology -

  • Monitoring of patients in active labor, induced labor, and obstetric emergencies.
  • Assisted in uncomplicated normal vaginal deliveries and C-sections, procedures like hysterectomy and sterilisation.
  • Postpartum ward monitoring of high-risk patients, including eclampsia, and postpartum hemorrhage.

5) Orthopaedics -

  • Daily dressings, wound care, limb strengthening and mobility exercises.
  • Participated in and assisted with POP, cast, slab applications, traction and stabilization.
  • Assisted in procedures including knee and hip replacements, nailing, plating, and arthroscopy.

6) Pediatrics -

  • Developed communication skills and empathy in interacting with children and their families.
  • Completed a one-week rotation in the NICU/PICU, observing and assisting with various critical care protocols, including neonatal resuscitation, and phototherapy.

7) Community Medicine :

  • Actively participated in rural outreach programs, health awareness drives, screening camps, and family adoption initiatives aimed at preventive care and community health education.

Peripheries as a part of the major rotations:

  • Neurology - Assisted in the care of stroke, ICH, and post-neurosurgical patients; performed systematic neurological examinations; interpreted CT/MRI brain imaging with seniors; and observed craniotomy procedures.
  • Cardiology - Attended one week of peripheral posting in cardiology, attending code STEMI with consultants, observing cath lab procedures, interpreting ECGs, and assisting in the acute management of MI.
  • Nephrology - Management of volume-overload patients, fluid therapy, diuretic management, and haemodialysis wards.
