- The NP is currently working for American health partners ; Truhealth of Indiana. The provider covers two facilities with at list 40 patients in managing their chronic and acute health care needs at their own facilities. This assures that pts are not disrupted by being taken to hospitals frequently for health conditions which could be otherwise be managed at their own facilities which is their home. NP makes sure that each pt is assessed once every month and visited with during every NP visit which is done twice a week Np completes a head to toe assessment, reviews their medications, diagnosis their medical conditions and where needed , refer them to specialist for a specialized care.
- NP coordinates care with nursing, primary care providers and the family to optimize the care. NP submits the notes for billing and for review of appropriateness of each code. The NP covers a weekly call and weekend calls per the schedule.
