Aim:  Evaluate the compliance of the Clinical Frailty Scale (CFS) completion on admission in Ward 14.

Key Actions : 

-Led data collection for the frailty scoring compliance audit, followed by a teaching session to improve documentation and raise awareness.

-Identified zero compliance in the initial cycle. 

-Conducted targeted teaching on frailty scoring and assessment tools to enhance awareness and promote adherence.

Aim : Enhance efficiency and continuity of care through improved handover processes.
