Intensive Case Manager, Addictions & Supportive Housing (ASH)

  • Provided holistic and client led case management from a Recovery/ Psychosocial Rehabilitation framework, to an assigned caseload of individuals living in independent ASH units located across Toronto
  • Developed collaborative relationships and worked in tandem with interdisciplinary staff from various agencies and institutions to deliver comprehensive wrap-around supports based on subjective needs
  • On-Site Task Supervisor for advance student placement for University Of Windsor
  • Supervised the student, based on university curriculum and experiential learning practices in the community
  • Advocated for clients at risk of eviction in their living environments, for a more integrated and equity based living, with building managements across Toronto
  • Worked on de-cluttering and self-advocacy in collaboration with VHA, for clients at high risk of eviction
  • Raised 1500$ funding in collaboration with RBC's Communities Together Fund (CTF), for Christmas 2024, to support members in the community, based on 'wants', instead of 'needs'
