Commute to different facilities in the state of Indiana for 8-12 weeks including acute, outpatient, and SNF settings to assist with treatment sessions. Document resident's progress or deficits according to departmental standards.
Communicate resident progress or problems with supervisor and other teams members to maximize rehab potential. Carries out treatment plans that's established by Registered Occupational Therapist for inpatients.
Facilitated ADLs and assisted patients with functional mobility to increase comfort and quality of life. Consulted with other direct-care staff and worked with caregivers to implement home programs that achieved occupational therapy goals and contributed to residents care plan meeting. Instructed residents on the use of adaptive equipment. Communicated residents progress or deficits to supervisor and other team member.
